Well they did tell us the boys relationship would get much worse before it got better so I'd not be surprised if the Unforgiven is Sam. But we all know Dean, no matter what Sam is still his brother and he will find a way to get the "real" Sam back.
Or maybe that's how Sam is gong to feel at that point in the season after getting back his soul which I suspect is going to happen mid-season. I believe souled Sam is going to be an emotional mess overcome by the personal toll of everything he did in the past year on top of his experiences in Lucifer's cage which I suspect were far from pleasant. I can see Dean fighting to prove to his brother that there is hope for hm and his future.
Unforgiven is also the title of a movie, and since they have used a lot of movie/tv show titles this year I bet it has something to do with this: http://en.wikipedia.org/wiki/Unforgiven
I agree with you. After reading the synopsis of the movie, maybe Bobby will be the focus and he will become really active for one more big job. He's lost his wife and most of his friends and never seems truly happy. He sits in his house and we never hear about him doing anything else. The one time he reached out to a woman, she turned against him after he saved his life. It was really sad.

Does anyone know anything more about the episode "Caged Heat"? I haven't seen anything since reading what the title of the episode was going to be.

I wish that when they gave episode titles, they'd give us a small synopsis along with it so we could get some sort of idea on what the episode might entail.

I hope TPTB don't kill him off because then I'll have to do some serious damage to Sera & Co............serious damage.......... - anyhoo - I was watching Dead Men Don't Wear Plaid this morning - at the end Bobby was telling the boys that Death had come for him then. Course he didn't get him that time, but something tells me Death wouldn't give up too easy. And we know Death is still out there, and is due to turn up very soon. Oh Sera pleeeaaaase don't kill Bobby!

Unforgiven... at least for me is such a strong word, I mean, I think that there are few things in life that are unforgiven...So I'm so excited about this I strongly believe that there is nothing unforgiven between the boys...I'm sure of it
It's possible. Sounds as good a theory as any. I wondered if that's where Sam's soul really is.
I'm not really into theology these days, and I was thinking - with all the debate going on as to what/where/if purgatory is - this is after all a) a TV show - so the theology can be whatever the writers want it to be; b) Supernatural - so see a) above.
Much as I love theorising, this season has so many possibilities it's giving me a headache. So I'm just gonna wait and see for now.

I had another thought about this title "unforgiven." In Catholicism there's an idea that if you confess your sins and do penance, unless they're mortal sins you'll be forgiven by God and get into Heaven. Now what if God has left for parts unknown? There would be no one to forgive people their sins, and all those people who would normally get into Heaven go to purgatory instead. It would explain why Crowley considers purgatory the hot new real estate market.
